UNDERSTANDING APPRAISAL

What does critical appraisal mean?

Critical appraisal is the process of examining research systematically rather than assuming that publication automatically makes a study strong, reliable or relevant.

For nursing students, appraisal helps you decide how useful a source is, what its findings can reasonably support and whether limitations should affect the strength of the claims you make in an assignment.

Identify what the researchers were trying to investigate.

Consider whether the methods were appropriate and sufficiently robust.

Decide how relevant the evidence is to your question or nursing context.

BEFORE YOU BEGIN

Appraisal is not the same as finding faults.

Critical appraisal should be balanced. A study may have important strengths while also having limitations that affect how confidently its findings can be interpreted.

The purpose is not to prove that every study is flawed. It is to judge what the evidence can reasonably tell you.

Eight questions to ask when appraising a journal article.

PURPOSE

What is the study trying to investigate?

Identify the research question, aim or hypothesis. A clear study purpose makes it easier to judge whether the design and methods are appropriate.

Ask: Is the research question clearly stated and relevant?

DESIGN

Is the research design appropriate?

Consider whether the study design matches the question being asked. Different questions may require qualitative, quantitative or mixed methods.

Ask: Does the chosen design make sense for the research aim?

SAMPLE

Who took part in the study?

Look at the sample size, participant characteristics, recruitment approach and whether the sample reflects the population the researchers are making claims about.

Ask: Is the sample appropriate and sufficiently representative?

METHODS

How was the information collected?

Consider whether the data-collection methods were suitable, consistent and capable of answering the research question.

Ask: Could the way data were collected introduce bias or error?

FINDINGS

What do the results actually show?

Separate what the study found from what the authors conclude. Check whether the conclusions are supported by the results presented.

Ask: Are the conclusions proportionate to the findings?

LIMITATIONS

What might weaken confidence in the findings?

Consider sample size, selection bias, missing data, measurement limitations, confounding factors and other weaknesses relevant to the study.

Ask: What could have influenced the results?

RELEVANCE

How applicable is the evidence?

Think about the setting, participants and healthcare context. Evidence from one population or healthcare system may not transfer perfectly to another.

Ask: How similar is this study to the context I am discussing?

OVERALL JUDGEMENT

What weight should you give the study?

Draw the appraisal together. Consider the study's strengths, limitations and relevance before deciding how strongly it should influence your academic argument.

Ask: How confidently can I use this evidence?
READ BEYOND THE ABSTRACT

The abstract is a summary, not a full appraisal.

An abstract can help you decide whether an article is relevant, but you usually need the full paper to judge how the research was conducted.

Examine the findings themselves rather than relying only on the conclusion.

DIFFERENT STUDIES, DIFFERENT QUESTIONS

Appraisal depends on the type of research.

The questions you ask will differ depending on whether the study is quantitative, qualitative or uses another design.

QUANTITATIVE RESEARCH

Think measurement and comparison.

  • Was the sample appropriate?
  • Were outcomes measured reliably?
  • Were groups comparable?
  • Were important confounding factors considered?
  • Are the statistical results meaningful?
QUALITATIVE RESEARCH

Think experience and interpretation.

  • Was the research question suited to qualitative enquiry?
  • Was participant selection appropriate?
  • Was data collection sufficiently detailed?
  • Was the analysis clearly explained?
  • Are the interpretations supported by the data?
THINK ABOUT BIAS

What might systematically influence the findings?

Bias does not simply mean that researchers were dishonest. It refers to systematic influences that may affect how participants are selected, information is collected, results are measured or findings are interpreted.

Selection bias

Are the people included in the study systematically different from the wider population?

Measurement bias

Could the tools or procedures used to collect data influence the results?

Recall bias

Are participants relying on memory that may be incomplete or inaccurate?

Researcher influence

Could interpretation or interaction affect how data are generated or analysed?

INTERPRETING RESULTS

Statistical significance is not the whole story.

A statistically significant result does not automatically mean that the difference is clinically important, large or useful in practice.

When relevant, consider the size of the effect, confidence around the estimate, the study population and whether the outcome matters in the real clinical context.

WORKED NURSING EXAMPLE

Appraising a study on structured handover.

Imagine you find a study reporting that use of a structured handover framework improved communication between healthcare professionals.

Purpose

First establish whether the study was designed to measure communication quality, errors, staff confidence or another outcome.

Methods

Consider the healthcare setting, sample, comparison method and how communication effectiveness was measured.

Findings

Check the size of the reported improvement and whether all relevant outcomes changed in the same direction.

Limitations

A small study from one ward, for example, may provide useful evidence while still limiting how confidently the findings can be generalised.

Overall judgement

You might conclude that the study supports structured handover while recognising that the strength and transferability of the evidence depend on the study design and context.

TURN APPRAISAL INTO WRITING

Do not leave your critical thinking in your notes.

The value of appraisal comes when it changes how you use the evidence in your assignment.

WEAKER

β€œSmith et al. (2024) found that structured handover improves communication.”

This reports the study's conclusion but does not show how critically the evidence has been considered.

STRONGER

β€œAlthough Smith et al. (2024) reported improved communication following structured handover, the single-site sample may limit the wider applicability of the findings.”

The evidence is being used while its limitations are also acknowledged.

COMMON MISTAKES

Judging only by the title

A relevant title does not guarantee that the methods or population fit your question.

Reading only the abstract

Important methodological details and limitations may be missing from the summary.

Assuming peer review means perfect evidence

Peer review is valuable, but published research still needs critical appraisal.

Finding one limitation and dismissing the study

Appraisal should weigh strengths and weaknesses rather than searching for one reason to reject evidence.

Strong evidence from a very different population may not transfer directly to your context.
