Recognising Hypercalcaemia for Student Nurses

Learn how to recognise a raised calcium result, identify dehydration, gastrointestinal and neurological warning signs, and understand when hypercalcaemia may require urgent clinical review.

Key principle: hypercalcaemia can cause few symptoms initially, but more significant or rapidly developing elevations may affect hydration, renal function, the nervous system and cardiac function.

What is hypercalcaemia?

Hypercalcaemia means that the calcium concentration in the blood is above the appropriate laboratory reference range. Clinical significance depends on the level, how quickly it has changed and whether symptoms are present.

Confusion or drowsiness New cognitive change, disorientation or reduced alertness. Neurological deterioration requires prompt clinical assessment.

Why might calcium become high?

Hypercalcaemia has several possible causes. Student nurses should focus on recognising abnormal results, relevant history and deterioration rather than trying to establish the diagnosis independently.

Parathyroid

Hormonal regulation

Primary hyperparathyroidism is one recognised cause of raised calcium.

Underlying illness

Other disease processes

Hypercalcaemia may also occur in association with significant underlying disease and requires appropriate investigation.

Medication & supplements

Relevant treatment history

Some medicines and supplements may influence calcium balance and should be included when communicating the clinical history.

Do not assume the cause from the calcium result alone. Hypercalcaemia requires appropriate clinical investigation and interpretation.

Confirm airway patency, particularly if consciousness becomes significantly reduced.

Assess respiratory rate, oxygen saturation and respiratory effort.

Circulation

Assess pulse, blood pressure, perfusion and possible evidence of volume depletion.

Assess consciousness and recognise new confusion, drowsiness or reduced responsiveness.

Exposure

Consider fluid balance, medication, relevant history and other signs of underlying illness.

Hydration

Why does fluid balance matter?

Hypercalcaemia can contribute to increased urine output and thirst. Continued water loss combined with reduced intake may lead to progressive dehydration.

Consider whether the patient is drinking adequately and whether nausea or illness is limiting intake.

Review urine output

Increased urination can contribute to ongoing water loss and should be considered within fluid balance.

Circulation

Look for dehydration

Pulse, blood pressure, perfusion and the patient's overall appearance can help identify worsening fluid depletion.

A cycle of deterioration can develop

Increasing urine output can worsen dehydration, while dehydration may contribute to further clinical deterioration. Recognising this pattern can help prompt timely review.

Cardiac awareness

Why can severe hypercalcaemia affect the heart?

Calcium contributes to normal cardiac electrical and muscular function. Significant abnormalities can alter cardiac electrical activity and may increase the risk of rhythm disturbances.

New palpitations, an abnormal pulse or cardiovascular deterioration should be communicated promptly.

  • Increasing weakness or fatigue.
  • New confusion or altered behaviour.
  • Increasing drowsiness or reduced responsiveness.
  • Evidence of worsening renal function.
  • New palpitations, rhythm concerns or cardiovascular instability.

Watch for progressive deterioration

Earlier

Mild elevation

Calcium is above the laboratory reference range but the patient is alert and relatively well.

Later

Symptoms increase

The patient becomes very thirsty, passes urine frequently and complains of weakness and constipation.

Now

Wider deterioration

Calcium has risen further and the patient appears dehydrated, increasingly confused and drowsy.

The trajectory matters

A rising calcium result combined with worsening dehydration and neurological change requires more urgent attention than an isolated mildly elevated result.

Putting the findings together

Example

A patient has had an elevated calcium result monitored during their admission.

Over the next day they become increasingly thirsty and begin passing urine much more frequently.

Their oral intake falls because they feel nauseated, and they now appear dry and weak.

Later, they become noticeably confused and unusually drowsy.

The concern is the combined pattern of rising calcium, increasing fluid loss, dehydration and neurological deterioration.

As a student nurse, recognise the change, assess the patient within ABCDE and promptly communicate the laboratory trend and clinical deterioration to the registered and medical team.

Common mistakes

Hypercalcaemia recognition errors to avoid

  • Looking only at the calcium result. Assess the patient and the trend.
  • Ignoring thirst and frequent urination. They can contribute to significant dehydration.
  • Missing constipation, nausea or weakness. These may contribute to the clinical picture.
  • Assuming confusion is unrelated. Neurological change requires assessment.
  • Failing to review fluid balance. Intake and urine output can reveal deterioration.
  • Ignoring renal-function changes. Kidney involvement may be clinically important.
  • Trying to independently determine or treat the cause. Hypercalcaemia requires appropriate clinical review.

Report the calcium trend and the patient change

Example escalation

“I'm concerned about Mr Green. His calcium has increased further from the previous result. He has become extremely thirsty and is passing much more urine. He now appears dehydrated and weak and has become increasingly confused and drowsy.”

This communicates the laboratory trend, fluid-balance change and neurological deterioration.
