Recognising Respiratory Fatigue for Student Nurses
Learn to recognise when a patient who has been working hard to breathe may be becoming exhausted, understand why an apparently quieter breathing pattern can sometimes be concerning, and know when urgent escalation is required.
What is respiratory fatigue?
Breathing requires muscular effort. When respiratory demand remains high, the muscles involved in breathing can become increasingly fatigued. A patient may eventually struggle to maintain effective ventilation.
Breathing becomes hard work
Illness can substantially increase the effort required to move air into and out of the lungs.
The patient works harder
Respiratory rate and muscular effort may increase as the patient tries to maintain adequate gas exchange.
Compensation can fail
If the effort cannot be sustained, breathing may become less effective and the patient can deteriorate rapidly.
From respiratory distress to respiratory fatigue

Signs that should increase concern
Exhaustion
The patient may look visibly tired after a prolonged period of difficult breathing.
Shallow respirations
Breathing may become less effective even if the respiratory rate appears lower.
Drowsiness or confusion
New neurological change alongside respiratory deterioration is particularly concerning.
Reduced ability to communicate
Increasing difficulty speaking or responding can accompany severe respiratory compromise.
Changing oxygen saturation
Interpret oxygen saturation against the patient's prescribed target, oxygen therapy and wider clinical condition.
The patient looks worse
Overall deterioration may be obvious even when an individual observation does not appear dramatically abnormal.
Do not assume a falling respiratory rate means improvement
Imagine a patient has been breathing rapidly and using considerable accessory muscle effort. Their respiratory rate then falls, but they become increasingly drowsy and their breathing appears shallow.
The respiratory rate must be interpreted alongside depth of breathing, effort, oxygenation, consciousness, symptoms and the direction of the overall clinical picture.
Respiratory fatigue requires a whole-patient assessment
Airway
Check airway patency and identify any immediate airway compromise.
Breathing
Assess respiratory rate, depth, effort, oxygen saturation, prescribed oxygen target and relevant respiratory findings.
Circulation
Assess pulse, blood pressure, perfusion and other relevant circulatory findings.
Disability
Pay particular attention to new confusion, drowsiness or reduced responsiveness.
Exposure
Consider temperature and other relevant findings while maintaining dignity.
Escalate
A patient showing signs of severe respiratory deterioration or exhaustion requires prompt escalation according to local emergency procedures.
The patient suddenly looks quieter
Example
Earlier in the shift, a patient with acute respiratory illness was sitting upright, breathing rapidly and using considerable effort to breathe.
When you reassess them, they appear quieter. Their respiratory rate has fallen, but their breathing is now shallow and they seem increasingly drowsy.
You recognise that this is not necessarily improvement. The combination of prolonged respiratory effort, shallow breathing and reduced alertness raises concern about respiratory fatigue.
You immediately begin an ABCDE assessment and obtain urgent senior help according to local escalation procedures.
Describe the change, not just the number
Example escalation
“I'm concerned about Mrs Green. She was previously breathing rapidly with marked respiratory effort. Her respiratory rate has now fallen, but her breathing is shallow and she has become drowsy. I'm concerned she may be deteriorating.”
This communicates the trend and the clinical concern, rather than presenting a lower respiratory rate as an isolated result.
Errors to avoid
- Assuming a falling respiratory rate always means improvement.
- Recording respiratory rate without assessing depth and effort.
- Ignoring increasing drowsiness or confusion.
- Looking only at oxygen saturation.
- Ignoring the patient's previous respiratory pattern.
- Waiting for every observation to become abnormal before escalating.
- Delaying urgent help while trying to establish a diagnosis.
