Recognising Unequal Chest Expansion for Student Nurses

Learn how to identify asymmetrical chest movement, connect it with other respiratory findings and recognise when an apparently small observation may form part of a more serious pattern of deterioration.

Clinical confidence principle: chest movement should never be interpreted in isolation. Compare both sides, consider the patient's baseline and connect the finding with breathing effort, breath sounds, oxygenation and symptoms.
The basics

What is chest expansion?

During inspiration, movement of the diaphragm and chest wall increases the volume of the thorax. In many patients, the two sides of the chest rise in a broadly symmetrical way.

Normal movement

Both sides usually move together

The chest wall should generally expand in a coordinated pattern, although normal variation exists between individuals.

Asymmetry

One side may move less

Unequal expansion means one side of the chest appears to move less than the other during breathing.

Context

It is a finding, not a diagnosis

Unequal expansion can occur for different reasons and requires interpretation within the wider clinical picture.

Observation

How might you recognise unequal chest movement?

Look at the patient

Observe the chest during normal breathing while maintaining privacy and dignity.

Compare both sides

Look for obvious differences in movement between the left and right sides of the chest.

Assess breathing effort

Consider respiratory rate, depth, accessory muscle use and whether the patient appears distressed.

Connect other findings

Consider breath sounds, oxygen saturation, pain, symptoms and the patient's recent clinical history.

Within competence: physical respiratory examination should follow your programme teaching, clinical supervision and local practice. Do not perform techniques you have not been trained to use.
Possible significance

Why might chest expansion become unequal?

Several different clinical situations can affect the movement of one side of the chest. The finding should prompt further assessment rather than an immediate diagnosis.

Pain

Movement may be limited

Pain from chest injury, surgery or another cause may lead a patient to reduce movement on one side.

Air movement

Ventilation may differ

Reduced movement can sometimes accompany reduced ventilation or changes within the affected side of the chest.

Underlying condition

Previous disease may matter

Surgery, chronic lung disease or structural differences may influence a patient's normal chest movement.

Do not diagnose from asymmetry alone. The useful question is whether the finding is new, whether the patient is unwell and what other respiratory changes are present.
Connect the findings

When should asymmetry increase your concern?

Associated finding What you may notice Why it matters
Sudden breathlessness A new or rapidly developing difficulty breathing. New asymmetry plus sudden respiratory symptoms requires prompt assessment.
Chest pain New pain, particularly if associated with respiratory distress. Pain may limit movement or form part of a more serious respiratory picture.
Reduced breath sounds Air entry may appear reduced on the same side. Several unilateral findings together increase the significance of the assessment.
Increasing respiratory rate The patient is breathing more quickly than previously. Tachypnoea may be an early indicator of worsening respiratory status.
Changing oxygenation Oxygen saturation differs from baseline or oxygen needs change. Interpret using the patient's prescribed oxygen target and wider assessment.
Overall deterioration The patient looks increasingly unwell. Clinical concern should prompt assessment even before every observation changes.

Assess the patient, not just the chest wall

A

Airway

Ensure the airway is patent and identify any immediate airway concern.

B

Breathing

Assess respiratory rate, depth, effort, chest movement, oxygenation and relevant breath sounds within your competence.

C

Circulation

Assess pulse, blood pressure and peripheral perfusion alongside the respiratory findings.

D

Disability

Identify new confusion, drowsiness, collapse or reduced responsiveness.

E

Exposure

Consider injury, surgery, wounds or other relevant findings while maintaining dignity.

Escalate

Seek prompt senior or emergency help according to local procedures if significant respiratory deterioration is suspected.

Clinical scenario

A patient suddenly becomes breathless

Example

A patient who had previously been comfortable suddenly reports increased breathlessness and right-sided chest discomfort.

Their respiratory rate has increased. When you observe their breathing, the right side of the chest appears to move less than the left.

You recognise that the unequal movement cannot identify the cause by itself. However, the combination of sudden breathlessness + chest discomfort + tachypnoea + new chest asymmetry is concerning.

You begin an ABCDE assessment and escalate promptly according to local clinical procedures.

Example escalation

“I'm concerned about Mr Evans. He has developed sudden breathlessness and right-sided chest discomfort. His respiratory rate has increased, and the right side of his chest appears to be expanding less than the left.”

This reports an objective finding and the associated deterioration without claiming an unconfirmed diagnosis.

  • Diagnosing the cause from chest asymmetry alone.
  • Ignoring whether the finding is new or longstanding.
  • Looking at chest movement without checking respiratory rate.
  • Failing to assess breath sounds and oxygenation where appropriate.

  • Performing examination techniques beyond your competence.
