Understanding ECG Monitoring for Student Nurses

Learn what bedside ECG monitoring can tell you, how to recognise meaningful changes and why monitor findings must always be connected with assessment of the patient.

Key principle: treat the patient, not the monitor. A monitor can alert you to a possible change in heart rate or rhythm, but the display must be checked against the patient's pulse, symptoms, observations and wider clinical condition.
The basics

What is ECG monitoring?

Continuous or bedside ECG monitoring displays the heart's electrical activity over time. It can help clinical teams identify changes in heart rate and rhythm and alert them to potentially significant events.

Rate

How fast?

Monitoring can provide ongoing information about the displayed heart rate and changes over time.

Rhythm

How regular?

Changes in the displayed rhythm may prompt closer assessment and, where appropriate, further ECG evaluation.

Trend

What has changed?

Monitoring can help identify new changes that may not be obvious from an isolated observation.

Important: bedside monitoring is not the same as a diagnostic 12-lead ECG. A monitor alert or rhythm change should prompt appropriate patient assessment rather than an immediate assumption about the diagnosis.
Monitor first principles

When the monitor changes, check the patient

Look at the patient

Determine whether the patient appears well, symptomatic or physiologically unstable.

Assess the pulse

Where appropriate, compare the displayed rate and rhythm with your clinical pulse assessment.

Check observations

Assess relevant physiological observations and look for evidence of wider deterioration.

Check the equipment

Consider whether movement, poor electrode contact or another technical issue could be affecting the display.

Compare with baseline

Establish whether the monitor finding is new and compare it with previous rhythm information where available.

Escalate appropriately

Communicate new or concerning findings according to the patient's condition and local escalation procedures.

Patient assessment

Ask what the patient is experiencing

The clinical importance of a rhythm change depends heavily on how the patient is responding to it.

Palpitations

Can they feel a change?

Ask whether the patient feels racing, pounding, fluttering or irregular heartbeats.

Circulation

Are they dizzy or faint?

Dizziness, near-syncope or collapse can increase concern about a cardiovascular change.

Chest

Any chest discomfort?

New significant chest pain or discomfort alongside a rhythm change requires prompt clinical assessment.

Breathing

Any breathlessness?

New or worsening breathlessness may indicate that the patient is not tolerating the change well.

Perfusion

How do they look?

Pallor, clamminess or other signs of altered perfusion can add to the clinical concern.

Consciousness

Are they alert?

New confusion, drowsiness or reduced responsiveness requires urgent wider assessment.

Technical awareness

Not every unusual trace is a cardiac event

ECG monitors can display artefact. Understanding this prevents you from responding to the screen without first checking the patient.

Movement

Patient movement

Movement, shivering and muscle activity can interfere with the displayed trace.

Electrodes

Poor contact

Loose or poorly contacting electrodes may produce misleading signals or monitor alarms.

Equipment

Lead problems

Disconnected or disturbed monitoring equipment can alter the displayed trace.

Never assume artefact without assessing the patient. A strange-looking monitor trace may be technical — or it may represent a genuine clinical change.

Connect ECG changes with the whole patient

A

Airway

Respond immediately to airway compromise according to local emergency procedures.

B

Breathing

Assess respiratory rate, oxygen saturation, work of breathing and respiratory symptoms.

C

Circulation

Assess pulse, blood pressure, perfusion and cardiovascular symptoms alongside the monitor.

D

Disability

Assess consciousness and recognise dizziness, syncope or new neurological change.

E

Exposure

Continue the wider assessment and identify other relevant clinical findings while maintaining dignity.

Escalate

Follow local emergency procedures immediately if the patient is significantly deteriorating.

Clinical interpretation

What should a student nurse notice?

Observation Useful question Student nurse response
Rate change Is the displayed heart rate significantly different from earlier? Check the patient, pulse and wider observations.
Rhythm change Does the rhythm look different from the previous display? Assess the patient and communicate a new change.
New alarm Why has the monitor alarmed? Check the patient before assuming the alarm is technical.
Irregular pulse Does clinical pulse assessment support a rhythm change? Report the finding for appropriate further assessment.
Symptoms Is the patient experiencing chest pain, breathlessness or dizziness? Escalate promptly when associated symptoms are concerning.
Instability Is there evidence of wider physiological deterioration? Use ABCDE and local emergency escalation procedures.
High-concern patterns

Monitor changes that become more concerning

  • A new rhythm change accompanied by significant chest pain.
  • A monitor change with severe or worsening breathlessness.
  • A rhythm or rate change associated with syncope or near-syncope.
  • Significant blood pressure deterioration alongside a monitor change.
  • New altered consciousness or reduced responsiveness.
  • Signs of poor peripheral perfusion with cardiovascular deterioration.
  • A persistent new monitor abnormality in a symptomatic patient.
  • Any monitor change accompanied by significant ABCDE deterioration.
The patient's condition determines the urgency. Do not delay emergency escalation because you are trying to identify or name the exact rhythm.
Clinical scenario

The monitor suddenly alarms

Example

You are caring for a patient receiving cardiac monitoring when the monitor alarms and displays a heart rate considerably faster than the previous trend.

Rather than focusing only on the screen, you immediately assess the patient. They report sudden palpitations and feel light-headed.

Their pulse is also fast and their blood pressure has fallen compared with the previous observation.

You recognise the concerning combination of monitor change + symptoms + pulse change + blood pressure deterioration, continue an ABCDE assessment and escalate promptly according to local procedures.

Communication

Describe what you see — without over-interpreting it

Example escalation

“I'm concerned about Mr Jones. His cardiac monitor has changed from the previous rhythm and is displaying a much faster rate. He is now reporting palpitations and light-headedness. His pulse is also fast and his blood pressure has fallen.”

This communicates the monitor change + clinical findings + symptoms + trend without requiring the student nurse to diagnose the rhythm.

Common mistakes

ECG monitoring errors to avoid

  • Looking at the monitor before looking at the patient.
  • Assuming every alarm represents a genuine rhythm change.
  • Assuming every unusual trace is artefact.
  • Trying to diagnose a rhythm beyond your competence.
  • Failing to assess the patient's pulse and wider observations.
  • Ignoring associated chest pain, breathlessness or dizziness.
  • Confusing bedside monitoring with a diagnostic 12-lead ECG.
  • Delaying escalation while trying to interpret the monitor yourself.
