Understanding Jugular Venous Pressure for Student Nurses
Learn what jugular venous pressure represents, how it contributes to cardiovascular assessment and why it should always be interpreted alongside symptoms, observations and the wider clinical picture.
What is jugular venous pressure?
Jugular venous pressure, usually shortened to JVP, reflects pressure within the right side of the circulation. Clinicians can estimate it by observing venous pulsation in the neck when the patient is positioned appropriately.
A window into circulation
JVP can contribute information about right-sided cardiac filling pressure and overall fluid status.
It is visually assessed
The venous pulsation is observed rather than palpated like an arterial pulse.
Never interpret it alone
Symptoms, blood pressure, pulse, oedema, breathing and the wider cardiovascular assessment all matter.
How is JVP observed?

Explain the assessment
Explain what you are observing, obtain consent and maintain the patient's dignity.
Position appropriately
Positioning affects the visibility of the venous pulsation, so follow the technique used within local practice.
Observe the neck
Look for venous pulsation rather than pressing on the vessel or confusing it with the carotid pulse.
Connect the finding
Interpret any abnormal appearance alongside symptoms, pulse, blood pressure, breathing and other signs of fluid status.
Do not confuse venous pulsation with the carotid pulse

What might a raised JVP mean?
A raised JVP can occur in several clinical situations. It should be treated as a finding that requires interpretation rather than as a diagnosis.
Possible congestion
A raised JVP may contribute to evidence suggesting increased venous pressure or fluid congestion.
Right-sided pressure change
Some cardiac conditions can increase right-sided filling pressures and may be associated with a raised JVP.
Look for associated findings
Oedema, breathlessness, blood pressure, pulse and overall clinical condition help determine the significance of the finding.
Connect JVP with other cardiovascular findings
- Pulse rate and rhythm.
- Blood pressure and trend.
- Peripheral oedema.
- Breathlessness or reduced exercise tolerance.
- Peripheral perfusion.
- Relevant fluid balance or weight trends where monitored.
- Chest discomfort, dizziness or syncope.
- The patient's usual baseline.
If the patient appears unwell, prioritise ABCDE
Airway
Identify airway compromise and obtain immediate help if required.
Breathing
Assess respiratory rate, oxygenation, breathlessness and work of breathing.
Circulation
Assess pulse, blood pressure, perfusion and relevant cardiovascular findings.
Disability
Consider consciousness, dizziness, syncope or new confusion.
Exposure
Look for oedema and other relevant findings while maintaining dignity and temperature.
Escalate
Follow local escalation procedures if the overall assessment indicates deterioration.
A patient is more breathless than yesterday
Example
A patient with a cardiac history reports increasing breathlessness and says their ankles have become more swollen over the last two days.
During a supervised cardiovascular assessment, the venous pulsation in the neck appears more prominent than expected.
Rather than concluding that the patient has a specific diagnosis, you connect the findings: increasing breathlessness + peripheral oedema + possible raised JVP + cardiac history.
You complete the appropriate wider assessment and report the change for further clinical review.
Describe the finding without over-diagnosing
Example escalation
“Mr Thompson is more breathless than yesterday and his ankle oedema appears worse. During supervised cardiovascular assessment his JVP also appeared elevated. His current observations are...”
This gives the receiving clinician the important findings without claiming a diagnosis that has not been established.
JVP assessment errors to avoid
- Using JVP alone to diagnose heart failure.
- Confusing jugular venous pulsation with the carotid pulse.
- Ignoring patient positioning.
- Focusing on the neck while ignoring the wider ABCDE assessment.
- Attempting advanced interpretation beyond your competence.
- Ignoring breathlessness, oedema or observation changes.
- Delaying escalation while repeatedly trying to identify the JVP.
