Why it matters

A specimen is only useful if it is collected correctly.

Clinical specimens are collected to provide information that can help with diagnosis, monitoring and treatment decisions. Examples include urine, stool, sputum, wound swabs and other samples requested as part of a patient's care.

Poor collection technique, incorrect labelling, contamination or delays in transport can reduce the quality of the sample and may lead to a misleading or unusable result.

Student nurses should therefore understand the whole process, not just the moment the sample is obtained.

Think of specimen collection as a chain: correct patient, correct sample, correct container, correct label and correct transport.

Prepare before obtaining the sample

Always follow the specimen collection procedure and laboratory guidance used by your organisation.

Understand which specimen is required, why it is being collected and whether there are specific timing or collection requirements.

Follow the required patient-identification procedure before collecting or labelling any specimen.

Follow local guidance and avoid unnecessary contact that could contaminate the specimen.

Secure the container

Close the specimen container correctly and check for leakage or contamination of the outside surface.

Label accurately

Follow local policy for patient identifiers, specimen details, date, time and collection site where required.

Store and transport appropriately

Send the specimen using the required packaging, transport system and timeframe so its quality is maintained.

Avoid touching the inside of specimen containers, lids or other protected parts unless the procedure specifically requires it.

Labelling

Correct labelling is a patient-safety issue

A well-collected sample can still become unusable or unsafe if it is labelled incorrectly.

  • Use the patient identifiers required by your organisation.
  • Check the specimen matches the intended patient before it leaves the bedside or collection area.
  • Record the specimen type and collection site where required.
  • Record the date and time of collection when required.
  • Complete electronic or paper laboratory requests accurately.
  • Never guess or correct unclear specimen details without following local policy.
Treat specimen labelling with the same care as medication identification: the result must belong to the right patient.
After collection

Finish the process safely

Dispose of waste

Dispose of used equipment using the appropriate clinical waste or sharps stream.

Perform hand hygiene

Complete hand hygiene at the appropriate point after specimen handling.

Document collection

Record the specimen and any relevant observations according to local practice.

Arrange transport

Place the specimen in the required packaging and send it through the correct transport pathway.

Report concerns

Escalate difficulty obtaining the sample, contamination, spillage or another collection problem when appropriate.

Follow up appropriately

Understand how results are reviewed and ensure urgent findings are acted on through the local clinical process.

Do not continue with a specimen if something about the request, patient, container or labelling is unclear.

Seek advice if you cannot confirm the patient, the requested sample is unclear, the specimen has been contaminated, the wrong container has been used, the label is incomplete or the sample has not been stored or transported correctly.

  • Learn where specimen containers are stored in your placement area.
  • Check which containers are used for common tests.
  • Practise explaining sample collection clearly to patients.
  • Learn the local labelling and laboratory request process.
  • Know how specimens are transported from your clinical area.
  • Ask for supervision whenever the collection technique is unfamiliar.
The goal is not simply to obtain a sample. The goal is to obtain the right sample from the right patient and make sure it reaches the laboratory in a condition that can produce a useful result.
